Our highly experienced team of clinicians, including nurses, physicians, nurse practitioners, sonographers, and genetic counsellors, is here to support you through your early pregnancy journey. We offer two programs to help you navigate the first trimester, no matter how your pregnancy is conceived.

What to Expect

A roadmap for the first trimester.

Every pregnancy is different, but most early pregnancies follow a familiar rhythm. Each milestone is supported by a clinician with full access to your care plan.

Weeks 4–6

Confirming the pregnancy

We confirm your pregnancy with an hCG (pregnancy hormone) blood test, then track the levels with serial blood work to make sure they’re rising as expected.

Week 7

Viability ultrasound

Your first ultrasound at the clinic confirms the location of the pregnancy and (for most) the first heartbeat. You meet with a nurse practitioner or physician the same day to review the results.

Weeks 8 to 10

Ongoing monitoringExtended Program

If you’re enrolled in the Extended Program, early pregnancy ultrasounds happen at the cadence your clinician recommends, each followed by an in-person visit. You’ll attend our prenatal screening webinar or meet one-on-one with a Twig genetic counsellor to learn about your options and determine which is right for you.

Early pregnancy loss is more common than most people realize, affecting roughly one in five pregnancies. It is rarely something you did or didn’t do. Whatever the path of your pregnancy, you stay with the team who has been with you.

Location

Compassionate management

If a loss is confirmed, your clinician walks through your options: waiting for the loss to complete naturally, medication to support the process, or D&C. We explain what to expect at each step, and the choice is yours.

On-site support

On-site medical and surgical management is available. Pain control, medications, and nurse practitioner-led follow-ups are coordinated through Twig, so you’re not navigating it alone.

Genetic testing of loss

Where appropriate, genetic testing after a pregnancy loss can help us understand what happened. Your fertility specialist reviews results with you and what they may mean for your next steps.

Reproductive counselling

Our partner Flourish × Twig offers private counselling sessions to help you process loss, grief, and what comes next. The first conversation is on us.

If you choose to try again, your specialist will discuss timing and any additional testing or care that may help. There is no pressure either way.

We offer two programs for your first trimester care. Every patient who does treatment at Twig is enrolled in the Standard Program. The Extended Program adds additional ultrasounds and visits, genetic screening counselling and coordination, and after-hours nurse support through the first trimester.

For patients who want longer, deeper support through the full first trimester.

Note: Genetic testing fees (NIPT, or genetic testing of a pregnancy loss) are not included in either program and are billed separately at the time of testing. In Ontario, in-clinic surgical management (D&C) of pregnancy loss involves additional fees, discussed with you at the time of care.

Yes. Twig offers comprehensive early pregnancy care, including blood work monitoring and a viability ultrasound around 7 weeks of gestation. We will make a referral to the pregnancy care provider of your choice and ensure a smooth handoff. We also offer an extended care program through the end of the first trimester, including additional ultrasounds, genetic screening and after hours nursing support.

As soon as you have a positive blood test through Twig. We begin monitoring your hCG levels and prepare for your first ultrasound around 7 weeks gestation.

No. Every patient at Twig is automatically enrolled when you’re eligible for early pregnancy care. In Ontario, the Standard Program is the default; the Extended Program is an optional add-on for deeper monitoring through the full first trimester. Self-Pay IVF patients receive the Extended Program at no additional cost as part of their treatment. In British Columbia, for patients arriving with a spontaneous pregnancy (not conceived through Twig), the full program (Standard plus Extended) is the only option. Patients conceiving through Twig treatment in BC follow the Ontario structure. Either way, we walk through what applies to your situation and any fees before they’re invoiced.

A viability ultrasound is performed in the clinic, typically around 7 weeks gestation (3 weeks after your positive pregnancy test). It confirms the location of the pregnancy in the uterus, the presence of a gestational sac and yolk sac, and (in most cases) a heartbeat. It is followed by an in-person visit with a nurse practitioner or physician to review the results.

In Ontario, eFTS and the NT scan are OHIP-covered. NIPT (non-invasive prenatal testing) is covered for some patients and self-pay for others. NIPT is more accurate and screens for more conditions. In BC, SIPS and IPS are MSP-covered, with NIPT self-pay for most patients. Our genetic counsellors host a weekly webinar to help you decide what’s right for you, and BC patients can also book a 1:1 session with a counsellor.

No. Genetic testing fees (NIPT, or genetic testing of a pregnancy loss) are billed separately at the time of testing. The Extended Program covers counselling, coordination, and post-test review.

Starting after your 7 week ultrasound, we initiate a referral to the obstetrician or family physician of your choice. Midwifery care is by self-referral. We send your records and results to your chosen provider and confirm the handoff before your last Twig visit.

Contact our nursing line as soon as you can. The next steps will depend on the specifics of your situation. For heavy bleeding, severe pain, fainting, or shortness of breath, go to the nearest emergency department and contact your care team after your arrival. Our after-hours nurse line is available outside clinic hours.

If you have a pregnancy loss while under Twig’s care, a Twig fertility specialist will walk you through all your options, from watchful waiting to medication or a procedure. We will work with you to coordinate the option that you choose including close follow up by phone and in person. We offer genetic testing of pregnancy losses for people who want more answers, and counselling services through our trusted partners to focus on your mental health. You deserve to feel supported and cared for during such a difficult time.

In most healthy first-trimester pregnancies, yes. Aerobic and strength exercise are encouraged. Sex is safe in the absence of bleeding or specific clinical concerns. Air travel is generally safe with appropriate precautions. Your clinician will let you know if anything in your specific situation calls for adjustments.
