Fertility Check
Curious about
your fertility?
A simple, low-pressure way to learn more about your reproductive health. One visit, one review, one personalized report.
For people with eggs
A quick, easy way to learn more about your reproductive health.
We assess your ovarian reserve using two simple tools: an AMH bloodwork test and a transvaginal ultrasound to count antral follicles. Together they give us a clear picture of your current egg supply.
Hormonal birth control can affect AMH and AFC results, sometimes underestimating both. Your specialist will help you interpret your numbers in that context.
What we test
Two simple measures.
Each measure is a piece of the picture. Together, they give you a clear, evidence-based baseline.
- Bloodwork
Anti-Müllerian Hormone
A blood marker that estimates how many eggs you have left.
AMH is produced by small follicles in the ovary. A single bloodwork sample, at any point in your cycle.
- Ultrasound
Antral Follicle Count
A live look at the small follicles in your ovaries right now.
Counted by transvaginal ultrasound. Together with AMH, AFC gives a clearer picture of your current egg supply. Ultrasound is OHIP-covered for Ontario residents.
What we test
Three simple measures.
Each measure is a piece of the picture. Together, they give you a clear, evidence-based baseline.
- Semen analysis
Sperm Quantity
Concentration, volume, and total motile count.
How many sperm are present and how many are moving as expected. Measured per sample.
- Semen analysis
Sperm Motility
How well your sperm move.
Progressive motility (forward movement) is the most important predictor of reproductive potential.
- Semen analysis
Sperm Morphology
The shape of individual sperm.
Sperm with normal shape are more likely to fertilize an egg. Reported as a percentage of the sample.
How it works
Three steps.

When does a Fertility Check make sense?
A Fertility Check is built for people who want clarity on their baseline, not patients who are actively trying to conceive.
- Planning a family in the next few years
- Not yet trying, but curious about your baseline
- On hormonal birth control and wondering what your numbers might look like
- Simply ready to know
- Patients actively trying to conceive without success
If you’ve been trying for 12 months (or 6 months if you’re 35+), a Family Building Consult is the better starting point. If you’re already a Twig patient, please contact your care team.

Important Context
What this test does and doesn’t tell you
Your Fertility Check results are not an indication of your ability to conceive. The numbers in your report reflect your ovarian reserve or sperm parameters on the date they are tested. Age and lifestyle are also factors to keep in mind. Repeated testing may help you understand how your numbers are changing over time.
Common Questions
Quick answers.
A Fertility Check at Twig is a two-step process. First, we measure your ovarian reserve using a blood test (AMH) and a transvaginal ultrasound (AFC). Then you meet virtually with a fertility specialist to review your results and any next steps.
Anyone interested in understanding their current fertility status can benefit from a Fertility Check, whether planning for the future or simply curious about their reproductive health.
No referral is required. You can book your Fertility Check directly online.
The baseline testing appointment typically takes about 15 to 30 minutes. For individuals with eggs, a transvaginal ultrasound and blood draw for AMH testing. For individuals with sperm, provision of a semen sample in a private collection room for analysis.
Your results will be reviewed with you during your virtual consult, scheduled approximately one week after your testing appointment. During this 20-minute session, a fertility specialist will explain your results and discuss next steps if any are recommended. If your results suggest further evaluation, your specialist may recommend a Family Building Consult or a deeper diagnostic workup. After your virtual consult, you’ll receive a personalized report summarizing everything.
No. A Fertility Check is a snapshot of your reproductive health at this moment, not a prediction of whether you can conceive. The results reflect your ovarian reserve or sperm parameters at the time of testing, and many other factors influence fertility. Your report is a starting point for understanding, not a verdict.
The frequency of Fertility Checks depends on individual circumstances. If you’re monitoring changes over time or planning for future family-building, your fertility specialist can advise on an appropriate interval for repeat testing.
A Fertility Check at Twig is $250 in Toronto and Vancouver, including baseline testing, your virtual specialist review, and a personalized report. The ultrasound portion is OHIP-covered in Ontario.
Yes, you can have a Fertility Check while on hormonal birth control; however, in some cases, hormonal birth control can impact your test results, making them appear lower than they may actually be. Be sure to inform your care team about all medications you are currently taking so they can interpret your results accordingly. In some cases, repeat testing after stopping hormonal birth control may be recommended.
AMH (Anti-Müllerian Hormone) is a hormone produced by the small follicles in your ovaries. Because the level of AMH in your blood reflects the number of follicles you have, it gives us a good estimate of your ovarian reserve, meaning the relative quantity of eggs remaining. AMH does not measure egg quality, which is largely determined by age, and it does not predict whether you will be able to conceive. It is one helpful data point that, alongside your AFC and your age, helps your fertility specialist build a picture of your reproductive health.
Both tests measure your ovarian reserve, just in different ways. AMH (Anti-Müllerian Hormone) is measured through a simple blood draw and reflects the hormone produced by your small ovarian follicles. AFC (Antral Follicle Count) is measured through a transvaginal ultrasound, where we count the visible follicles in your ovaries. At Twig, we do both as part of your Fertility Check. Together, they give us a more complete and reliable picture of your ovarian reserve than either test on its own.

A Fertility Check at Twig is a two-step process. First, we run a full semen analysis to measure sperm count, motility, and morphology. Then you meet virtually with a fertility specialist to review your results and any next steps.

A semen analysis gives us a detailed look at the health of your sperm. We assess the number of sperm in your sample (count), how well they move (motility), and how they are shaped (morphology). Together, these measurements help us understand your sperm parameters and what your results could mean for your family-building plans.
For the most accurate results, we ask that you abstain from ejaculation for two to five days before your appointment. It also helps to stay well-hydrated, avoid alcohol in the days leading up to testing, and let your care team know about any medications you are taking. You will produce your sample in a private collection room at our Toronto or Vancouver location.
Yes, though the effect is more gradual than it is for eggs. Sperm count, motility, and DNA quality can decline with age, and these changes can affect both the time it takes to conceive and the health of a pregnancy. Your specialist can walk you through what your results mean in the context of your age and your goals.
A result outside the typical range does not mean you cannot have a child. Sperm parameters can change over time and are influenced by many factors, including recent illness, stress, medications, and lifestyle. During your virtual review, your fertility specialist will explain your results, discuss what may be contributing, and walk you through any next steps, including the option to repeat testing.
